Our take on Provation

Provation makes procedure documentation software that replaces dictation and generic EHR templates with structured notes, scope image capture and automatic coding. Provation MD runs on-premises; Provation Apex is a cloud subscription. It suits hospitals, ASCs and larger GI groups. It is not a full EHR, and pricing is quote-only, so small practices may find it more than they need.

Who Provation is best for

Provation, founded in 1994 and based in Minneapolis, sells procedure documentation for gastroenterology, pulmonology and, since 2026, orthopedic surgery. Physicians build a note by clicking through a guided navigation tree of findings rather than dictating; the software assembles the report, pulls in scope images and generates CPT and ICD-10 codes. The note is then sent to the hospital or ASC's EHR. Provation is part of Fortive, which acquired it in 2021, and the company says it serves 43 of the top 50 U.S. hospitals for GI.

Best for

  • Hospital endoscopy units and ambulatory surgery centers with high procedure volume
  • GI and pulmonary groups that want structured, billing-ready procedure notes
  • Orthopedic ASCs wanting voice-driven operative notes (Provation Apex Ortho)

Not ideal for

  • Small office-based practices that need a full ambulatory EHR with scheduling and billing
  • Practices outside GI, pulmonary or orthopedic surgery
  • Buyers who want published, self-serve pricing

Provation pros and cons

Pros

  • Structured notes from a guided navigation tree, built on GI content the vendor says it has developed over 25 years
  • Direct image capture from endoscopy scopes into the procedure note
  • Automatic CPT and ICD-10 code generation from documented findings
  • Over 100 standard reports for benchmarking in Provation MD
  • Provation Apex runs on Microsoft Azure and is covered by SOC 2 and ISO 27001 certifications, per the vendor

Cons

  • Quote-only pricing; no public rates
  • A documentation tool, not a full EHR; it relies on the facility's EHR for scheduling, orders and billing
  • Provation MD requires on-premises servers and local IT support
  • Specialty coverage is narrower than general EHR templates

Provation pricing

Provation does not publish prices. Provation MD is an on-premises, server-based license, and Provation Apex is sold as a cloud subscription. Pricing typically depends on the product, the number of physicians and procedure rooms, specialties covered, interfaces needed and implementation services.

Key features

  • Provation MD: on-premises documentation for high-volume and complex GI and pulmonary procedures, with 100+ standard reports
  • Provation Apex: browser-based cloud documentation for GI (Apex GI), pulmonology, nursing (Apex Patient Charting) and orthopedics (Apex Ortho)
  • Provation Mira Documentation Assist: voice-driven documentation within the cloud GI platform, announced in March 2026
  • Voice-to-note for orthopedics: turns dictation into a structured, billing-ready operative note, with DICOM integration for arthroscopic imaging
  • Scope image capture into procedure notes
  • Automatic CPT and ICD-10 coding
  • Related products: Provation iPro (anesthesia), Quality Concierge, Order Set Advisor and Care Plans

HIPAA, security and integrations

Provation's Apex cybersecurity fact sheet says Apex runs on Microsoft Azure with geo-replicated data, is monitored 24/7 by Provation's security operations center, and holds SOC 2 and ISO 27001 certifications; the sheet also displays SOC 3, ISO 27701 and HIPAA marks. Provation MD is hosted on the customer's own servers, so its security depends on the facility's infrastructure. BAA terms are not published.

Provation says Apex has pre-built integrations with Epic, Cerner (Oracle Health) and MEDITECH. Its HL7 fact sheet for Provation MD and MultiCaregiver describes outbound ORU messages for procedure notes and DFT for charges, and inbound ADT, SIU, ORM and MFN messages for patient demographics, schedules, orders and provider data. Provation says its professional services team has deployed over 6,000 HL7 interfaces.

Provation alternatives

  • Epic — many hospitals document procedures in Epic's own templates instead of a separate tool.
  • ModMed — specialty EHRs, including gastroenterology, for office-based practices that want EHR, PM and billing together.
  • NextGen Healthcare — an ambulatory EHR for specialty groups that want one system across clinic workflows.
  • Dragon Medical One — cloud speech recognition for physicians who prefer dictating notes into the EHR.

Provation FAQ

Is Provation good for small practices?

Provation is aimed mainly at hospitals, ambulatory surgery centers and larger GI and pulmonary programs. A small office-based practice would still need a separate EHR for scheduling, charting and billing, and Provation pricing is quote-only. Small GI groups that perform procedures at an ASC may still use Provation there, often through the facility.

How much does Provation cost?

Provation does not publish pricing. Provation MD is an on-premises license and Provation Apex is a cloud subscription. Cost depends on the product, number of physicians and procedure rooms, specialties, EHR interfaces and implementation services. Ask Provation for a quote and use its ROI calculator to estimate savings against dictation and transcription.

Is Provation HIPAA compliant?

Provation says Provation Apex goes beyond HIPAA requirements, runs on Microsoft Azure and holds SOC 2 and ISO 27001 certifications. Provation MD runs on the customer's own servers, so the facility controls much of the security environment. Request a business associate agreement and current audit reports from Provation as part of contracting.

What EHRs does Provation integrate with?

Provation says its cloud platform has pre-built integrations with Epic, Cerner (Oracle Health) and MEDITECH, among others. Its HL7 interfaces send finished procedure notes and charge data to the EHR and receive patient and order information. Ask Provation which interface method applies to your EHR version.

What is the difference between Provation MD and Provation Apex?

Provation MD is on-premises, server-based software for high-volume GI and pulmonary programs, with more than 100 standard reports. Provation Apex is cloud-based and accessed in a browser on subscription, with lower IT overhead and newer features such as voice-driven documentation. Apex also covers orthopedics and nursing documentation.

Provation vs Epic: do I need both?

Many hospitals run Epic as their EHR and use Provation for procedure notes, sending the finished report and charges back into Epic. Provation offers structured GI content, scope image capture and automatic coding that general EHR templates may lack. If your Epic procedure templates already meet your reporting and coding needs, a separate tool may not be necessary.
