Who WaiverPad is best for
WaiverPad, part of the KiteMD group of products, is a library of 185 ready-written liability waivers and consent forms across 11 industries. The healthcare-adjacent part is its Med Spa set (30 forms for treatments such as injectables, chemical peels, lasers and body contouring) and its Wellness set (32 forms for services such as IV drips, massage, acupuncture, cryotherapy and cold plunge). Each form is customized with the business name, address and state, and either downloaded as a PDF or signed online by clients on their phones.
Best for
- Med spas and aesthetic practices that want treatment-specific consent and liability forms quickly
- Cash-pay wellness businesses such as IV hydration, massage, recovery and bodywork studios
- Small businesses replacing paper waivers with phone signing at a low monthly cost
Not ideal for
- Practices covered by HIPAA that need consent forms stored with patient records
- Anyone who needs attorney-reviewed documents out of the box
- Businesses that want to upload or heavily edit their own consent text

- Each signed PDF ends with an audit page recording who signed, when, from which device, and a fingerprint of the file
- Governing-law wording for all US states and DC, Canada, Australia and the UK

- No form is marked attorney-reviewed yet; every page carries a "Template" badge and a have-your-counsel-review-it notice
- You cannot upload your own waiver text for online signing
- No EHR, booking or practice management integrations are published

HIPAA, security and integrations
WaiverPad states that online signing is not a HIPAA service and that its forms do not collect medical history. It does not offer a business associate agreement, so it is not the right place for protected health information. Practices and med spas that bill health insurance electronically are generally HIPAA covered entities and should keep clinical consent and medical history in their EHR or a HIPAA-compliant intake tool. WaiverPad publishes no integrations. Clients sign from a link or QR code, and PDFs can also be printed or sent through an e-signature tool you already use.
WaiverPad is not a law firm, and its documents are templates rather than legal advice. Have an attorney licensed in your state review any waiver before you rely on it.
WaiverPad alternatives
- Consent forms in your EHR or practice management system — the best choice for HIPAA-covered practices, because consent stays with the patient record. See EHR software.
- Digital intake tools in patient engagement software — often handle intake forms and consent alongside reminders and scheduling, with a BAA.
- Compliancy Group — for a full HIPAA and OSHA compliance program rather than client waivers.
