Our take on Paubox

Paubox adds HIPAA-compliant encryption to an existing Google Workspace or Microsoft 365 mailbox, so staff send PHI from their normal inbox and recipients read it without a portal or password. It suits small and mid-sized practices that email patients and partners often. The trade-off is scope: it secures email, forms and outreach, not your wider IT or compliance program.

Who Paubox is best for

Paubox sells HIPAA-compliant email to healthcare organizations. Its Email Suite encrypts outbound mail by default from Google Workspace or Microsoft 365, and recipients read messages in their usual inbox. Paubox says more than 9,000 healthcare organizations use it.

Best for

  • Small and mid-sized practices already on Google Workspace or Microsoft 365
  • Clinics that email patients, labs and referral partners and want to avoid portal logins
  • Organizations that want inbound phishing and spoofing protection from the same vendor
  • Digital health teams that need a HIPAA-compliant transactional Email API

Not ideal for

  • Practices on other email platforms
  • Teams that want secure texting, fax or telehealth rather than email
  • Organizations needing a full HIPAA compliance program (risk assessments, policies, training)

Paubox pros and cons

Pros

  • Encrypts outbound email by default with no recipient portal, password or plugin
  • Published pricing and a 14-day free trial
  • Business associate agreement included with every account at no extra charge
  • HITRUST r2 certification, valid through 2027 per the vendor
  • HIPAA-compliant forms included free with paid plans

Cons

  • Five-sender base pricing means very small offices pay for senders they may not use
  • If a recipient's server lacks modern TLS, the message arrives as a secure link instead
  • Inbound security, archiving and DLP require the Plus or Premium plans

Paubox pricing

Paubox publishes Email Suite pricing per sender (one email address). As of October 2026, the base bundle of up to 5 senders costs:

  • Standard: $32 per month billed annually ($41.60 monthly). Default outbound encryption and HIPAA-compliant forms.
  • Plus: $65 per month billed annually ($84.50 monthly). Adds AI inbound security, malware protection and ExecProtect+ display name spoofing protection.
  • Premium: $75 per month billed annually ($97.50 monthly). Adds archiving and data loss prevention.

Extra senders are priced individually (for example, $10.50 per sender per month on Standard, billed annually, for senders 6 to 50). Paubox Marketing and the Email API are priced separately.

Key features

  • Paubox Email Suite: outbound encryption by default using TLS 1.2 or higher
  • Inbound Security: AI-based protection against phishing, spoofing, spam and ransomware
  • Data Loss Prevention: blocks emails with sensitive data, including attachments
  • Archiving: search, view and export archived email for audits
  • Paubox Forms: HIPAA-compliant patient intake forms
  • Paubox Marketing: HIPAA-compliant email campaigns
  • Paubox Email API: transactional email for developers
  • Security Awareness Training: a newer add-on for staff training

HIPAA, security and integrations

Paubox includes a BAA with all accounts at no additional charge. It holds a HITRUST r2 certification, first earned in 2019 and renewed in 2025, which the vendor says is valid until 2027; the certification letter describes the Paubox web app as hosted on Amazon Web Services. Paubox works with Google Workspace and Microsoft 365, and its Email API lets other software send encrypted transactional email. It does not integrate with EHR charts directly.

Paubox alternatives

  • Spruce Health — secure messaging, phone, fax and video in one app instead of encrypted email.
  • XenFax — HIPAA-compliant cloud fax for practices that still exchange records by fax.
  • Compliancy Group — HIPAA compliance program software and coaching rather than an email tool.
  • Accountable — HIPAA risk assessments, policies and staff training for small practices.

Paubox FAQ

Is Paubox good for small practices?

Yes, for small practices that use Google Workspace or Microsoft 365 and email PHI regularly. It sits on top of the existing mailbox, so staff keep their workflow and patients need no portal login. The drawback for a very small office is the five-sender base bundle: a two-person practice pays the same base price as a five-person one.

How much does Paubox cost?

Paubox Email Suite starts at $32 per month billed annually for up to five senders on Standard, or $41.60 billed monthly. Plus costs $65 and Premium $75 per month on annual billing for the same five senders, and extra senders are priced individually. Every plan includes a BAA and a 14-day free trial.

Is Paubox HIPAA compliant and does it sign a BAA?

Paubox provides a business associate agreement with every account at no extra cost and holds a HITRUST r2 certification that it says is valid until 2027. Outbound email is encrypted by default with TLS 1.2 or higher. Your practice still needs its own HIPAA policies and staff training, because Paubox secures only the email channel.

Do Paubox recipients need a password or portal?

Usually not. Paubox delivers encrypted email straight to the recipient's inbox, so patients read it like any other message without an account. If the recipient's mail server does not support modern TLS, Paubox sends a secure link to open the message instead. This is the main difference from portal-based encrypted email services.

Paubox vs Spruce Health: which should a practice use?

They solve different problems. Paubox secures the email staff send from Google Workspace or Microsoft 365, with inbound threat protection on higher plans. Spruce Health combines secure messaging, a phone system, fax and video visits in one app. Practices that rely on email lean toward Paubox; those moving patient communication to texting and calls lean toward Spruce.
