Who athenahealth is best for
athenahealth sells athenaOne, a single cloud platform that combines the EHR (formerly athenaClinicals), practice management and billing (formerly athenaCollector), and patient engagement (formerly athenaCommunicator). Every customer runs the same version, and updates arrive automatically in seasonal releases. This page covers athenaOne as an EHR and practice platform; for the billing side, see our athenahealth RCM review.
Best for
- Small and mid-sized independent primary care and specialty practices
- Practices that want patients to self-schedule, check in, pay and message from an app
- Groups that want EHR, scheduling and billing from one vendor without running servers
- Practices that want a choice of ambient AI note tools built into the EHR
Not ideal for
- Very small or low-revenue practices that prefer a flat monthly fee
- Practices that need extensive custom templates or workflows
- Behavioral health or therapy practices that need a specialty-built system
athenahealth pros and cons
Pros
- One cloud system for charting, scheduling, billing and patient communication
- Patient self-scheduling, self check-in with forms and consents, online bill pay and the athenaPatient app
- athenaAmbient, a built-in ambient scribe, is included at no added subscription cost
- Ambient Notes lets clinicians choose AI documentation partners, with Abridge, Suki and Microsoft Dragon Copilot announced as options
- Large Marketplace of third-party apps that integrate with athenaOne
Cons
- Percentage-of-collections pricing can cost more than flat-fee EHRs as revenue grows
- Users frequently report long phone support waits, though live chat is often praised
- Reviewers describe click-heavy screens and a learning curve for working claims
- Limited customization; updates apply to everyone whether or not they suit your workflow
athenahealth pricing
athenahealth does not publish athenaOne prices as of 2026. Pricing is quote-based and is described by athenahealth as a percentage of monthly collections. Third-party estimates, such as IntuitionLabs', put that percentage at roughly 4% to just over 10%, but the actual rate depends on specialty, practice size, collections volume and services included. Ask for implementation fees, Marketplace app costs and contract length in writing.
Key features
- athenaOne EHR: specialty templates, e-prescribing, orders, results, quality reporting and embedded telehealth
- Practice management: scheduling, eligibility, registration, time-of-service collections and copay estimates
- Patient engagement: athenaPatient app, patient portal, self-scheduling, self check-in, automated reminders, Patient Conversations two-way texting, and a waitlist that texts patients when slots open
- AI tools: Ambient Notes (a choice of partner scribes), athenaAmbient (athenahealth's built-in ambient scribe, launched August 2026) and an AI-native clinical encounter announced in November 2025
- Other lines: athenahealth also supports older products such as athenaPractice and athenaIDX, which came largely from its combination with Virence Health, the former GE Healthcare clinical business solutions (Centricity) division
HIPAA, security and integrations
athenaOne is cloud-hosted by athenahealth, so the vendor runs servers, backups and updates. athenahealth operates a Trust Center where clients request its current security certifications, attestations and audit reports; the specific certifications and BAA terms are not published. Practices should request the latest SOC report and confirm BAA coverage for any Marketplace apps they add, since those are separate vendors.
Integration runs mainly through the athenahealth Marketplace, where partner products connect to athenaNet, athenahealth's network platform. Examples include Suki's ambient scribe (generally available inside athenaOne's Ambient Notes since May 2025), Microsoft's Dragon tools and Dolbey's Fusion Narrate dictation.
