Key differences
Primary care focus versus broad ambulatory coverage
Elation builds only for primary care: family medicine, internal medicine, pediatrics, women's health, geriatrics and direct primary care. Its charting, orders, referrals and results are designed around that work, and it holds a Best in KLAS 2026 badge for small-practice ambulatory EHR. athenaOne serves primary care and many specialties on one platform. Surgical or procedure-heavy practices will usually find more fit in athenaOne.
Pricing model
This is often the deciding factor. Elation publishes its prices. As of November 2026, its product guide lists Standard at $425 per provider per month, $375 for direct primary care providers and $215 for non-prescribing providers, with office staff included. Premium is $500. EHR plus billing bundles cost $699 or $799 per provider. athenahealth does not publish prices and bases fees on a percentage of monthly collections. IntuitionLabs estimates the range at about 4% to just over 10%. For a practice collecting $40,000 a month per provider, 5% would be $2,000, well above Elation's bundle. For a low-revenue or DPC practice, Elation's flat fee is easier to predict. Ask athenahealth for your rate and what services it covers.
Billing and revenue cycle
athenaOne puts scheduling, eligibility, claims and patient payments in the same system as the chart, with athenahealth's revenue cycle tools behind it. Elation sells billing in bundles that add real-time eligibility, claims processing and ERA posting, and it can also connect to Kareo billing for $40 per practice a month. Direct primary care practices that bill memberships rather than insurance can use Elation's membership management instead, which charges 2.99% of card payments and requires a Stripe account.
AI documentation
Both include an ambient scribe. athenahealth made athenaAmbient generally available across athenaOne in August 2026 at no added cost, and also lets clinicians use partner scribes. Suki's ambient notes became generally available inside athenaOne in May 2025. Elation's Note Assist drafts notes from the visit using chart context. Each provider gets 10 AI notes a month at no extra cost, and the Elation AI add-on is $199 per provider a month for more. Heavy scribe users should factor that into Elation's price.
Patient engagement
athenaOne has the stronger patient self-service set: the athenaPatient app, where patients can self-schedule, check in and complete paperwork before arriving, message the care team, view results and join virtual visits, if the practice turns these on. Elation offers the Patient Passport portal and integrated telehealth at $25 per provider a month.
Which should you choose?
For primary care, compare total annual cost under each model at your real collections. Elation usually wins on predictability and charting speed. athenaOne wins on breadth, billing depth and patient self-service.
Choose Elation Health if:
- You are an independent family, internal medicine, pediatric or direct primary care practice
- You want published flat pricing that does not grow with collections
- You want a chart-centered EHR and are fine adding billing as a bundle
Choose athenahealth if:
- You are a multi-specialty or specialty group, or expect to add specialties
- You want EHR, practice management, billing and patient self-service in one system
- You want an ambient scribe included without a per-provider add-on fee
Practices weighing other options can compare eClinicalWorks, which uses per-provider pricing, or Practice Fusion for a lower-cost EHR.
